To participate in IV-E Rate Setting, agencies must attend training and submit the following information to be received by the Office of the Controller no later than March 17, 2008:

  • Residential Treatment and Foster Care Cost Report

  • Documentation of All Child Care Days

  • An Independent Financial Audit

  • Time Study

Prior to applying for IV-E Foster Care rates, new agencies must qualify through and have a current Memorandum of Agreement (MOA) with the Division of Social Services (DSS) and participate in IV-E rate setting. For more information on DSS requirements, please contact DSS Financial Resource Coordinator, Tina Bumgarner.

Documents can be submitted electronically or in hard copy to one of the Foster Care Rate Setting addresses.

Training will be provided at several sites across the state during January 2008. Agencies are required to attend training and since seating is limited, it is necessary to register in advance. To register for training, select Training Schedule and Registration, to view the schedule and download the registration form.

Cost Report Forms and Instructions can be downloaded and printed. Forms are in Microsoft Excel and can be completed and submitted electronically.

After the completion of training, and submission of required reports, providers are asked to provide feedback by completing the Residential Treatment and Foster Care Cost Reporting Survey.
