Cost Report and Time Study Forms and Instructions

Automated processing includes forms and schedules created in Excel. Use of these automated forms and schedules provide time saving computations, linked schedules with data totals carried forward to the next document, and the flexibility to electronically submit the cost report data file, supplemental forms and workbook(s) via email.

Annual Time Study

The time study entails the completion of the Workers Daily Report of Services Provided to Clients. They must be completed in accordance with the Specific Instructions provided to ensure accuracy and thoroughness. Agencies must send the completed time sheets for the entire month of February 2008, to be received in the Office of the Controller by March 17, 2008.

Cost Report Forms and Instructions

There are two versions of the cost report for the 2008-09 rate setting year. The Residential Treatment and Foster Care Cost Report has additional columns and schedules needed to collect the information needed for DMA Rate Setting on residential mental health treatment services. Agencies that do not provide mental health residential treatment services can use the Foster Care Cost Report which does not contain the additional columns and schedules used to report residential mental health treatment. The User’s Manual contains instructions for both reports and the time study.
