The CON Section has revised the certificate of need application for New Operating Room Projects, New Ambulatory Surgical Facilities and Gastrointestinal Endoscopy Procedure Room Projects that must be used by all applicants for the development of new operating rooms, including the relocation of existing operating rooms, new ambulatory surgical facilities and new gastrointestinal endoscopy rooms.
This application replaces all previous New Operating Rooms, New Ambulatory Surgical Facilities and Gastrointestinal Endoscopy Procedure Room applications and must be used for all reviews beginning on or after September 1, 2008.
Prospective applicants are encouraged to contact the Certificate of Need Section to schedule a preapplication conference with the project analyst who is assigned to the county for which you are applying. The conference should be scheduled well in advance of the date the application is to be filed. Please do not hesitate to call the project analyst if you have any questions regarding how to complete the revised application.
