The CON Section has revised the Certificate of Need Application Form for Acute Care Facilities, Services and Medical Equipment. This application must be used for all projects except adult care homes, nursing homes, life care facilities (CCRCs), inpatient rehabilitation beds or facilities, intermediate care facilities for the mentally retarded, chemical dependency treatment beds or facilities, end stage renal disease facilities, home health agencies, hospice projects, and ambulatory surgery and operating room projects.
This application replaces all previous Acute Care Facility / Medical Equipment Project applications and must be used for all reviews beginning on or after September 1, 2008.
Prospective applicants are encouraged to contact the Certificate of Need Section to schedule a preapplication conference with the project analyst who is assigned to the county for which you are applying. The conference should be scheduled well in advance of the date the application is to be filed. Please do not hesitate to call the project analyst if you have any questions regarding how to complete the revised application.
