Money Follows the Person Project

Money Follows the Person (MFP) is a rebalancing initiative to help some institutionalized individuals with developmental disabilities, nursing care needs, or behavioral health needs move back into the community.

Providers, caregivers, advocates, and consumers may also participate in an ongoing statewide advisory committe. To be considered for participation, please complete and submit the Request for Appointment form.

DMA received notice of the grant award in a letter from CMS dated May 3, 2007. DMA formally accepted the grant award in a letter to CMS dated May 21, 2007.

The MFP Rebalancing Demonstration - Operational Protocol Instruction Guide (January 2007) is the official instruction guide and template for the development of Operational Protocols by the states chosen to participate in the Money Follows the Person (MFP) Rebalancing Demonstration.
