Kindergarten Health Assessments (PDF, 703 KB) En Español (PDF, 936 KB) measure the health of all children entering kindergarten in North Carolina. Health issues that affect learning are addressed.
State law requires that every child entering kindergarten in an N.C. public school receive a health assessment. The assessment must occur within 12 months prior to entering school. The medical provider, parent or guardian must provide a completed health assessment transmittal form to the principal of the school on or before the child's first day of attendance.”
Many local health departments and physicians across the state perform the assessment.
All children are required to receive a Kindergarten Health Assessment prior to entering kindergarten
Your child can get a kindergarten health assessment at a participating local health department or at your doctor’s office. Call for an appointment. Payment may be determined by income and insurance type.
