Call to Order of the State Rehabilitation Council
Following the Commission meeting, Tom Winton opened the State Rehabilitation Council meeting at 11:20 a.m. The following members were present: Allen Moore, Mary Le O’Daniel, Ron Huber, Terry Wethington, and Kathy Brack.
Members not present: John Miller, Norma Krajczar, Annette Clinard, Catherleen Thomas, Anita Wayne, and George Murphy. All were excused absences.
Others present: Debbie Jackson (ex-officio), Emily DuBois, Sandy Foster (ex-officio), David Arthur, John DeLuca, and Sally Syria.
Approval of Minutes
Motion was made by Mary Le O’Daniel, seconded by Terry Wethington, and carried to approve the minutes of the September 13, 2003 meeting.

Annual Report
David Arthur gave an update. The report is pretty much complete. Mr. Arthur stated that he is waiting on the message from the Chair, John Miller. Two price quotes have been obtained for printing. A draft copy has been taken to DHHS for approval. The Annual Report should be out by the end of the year.
Satisfaction Survey
Mr. Arthur handed out information regarding the satisfaction survey.
Mr. Arthur indicated that the DSB Rehabilitation Program, in conjunction with the State Rehabilitation Council, conducted a consumer satisfaction survey of individuals exiting the rehabilitation program during the State Fiscal Year 2002-2003. Surveys were mailed to 673 individuals whose cases closed with an employment outcome (status 26). The US Postal Service returned 34 surveys and 243 survey (38%) were returned completed.
Unsuccessful closures will be addressed again in 6 months.

With no further business to come before the State Rehabilitation Council, the meeting was adjourned at 11:50 a.m. The next meeting is scheduled for March 13, 2004.
