To request information from the North Carolina Division of Social Services, please refer to the following:
Adam Walsh Clearances
To obtain child abuse and neglect clearances to approve an individual to become a foster parent or to adopt, please complete the DSS-5268.
Once completed the DSS-5268 may be submitted by mail to:

Assistance Verifications for Out of State Agencies
If you are requesting out-of-state verification of assistance, please do
not send confidential information via email. You may obtain verification by calling 866-719-0141 or faxing a request to 252-789-5395.The request
should be on agency letterhead and must include:
- Name
- Date of Birth
- Social Security Number
- Programs included in request
- Your agency's contact information
Child Protective Services Requests
If you are requesting information in response to an open case involving Child Protective Services, please submit DSS-5277. You may submit this request by fax to (919) 715-6714 or by mail to:

Non-Identifying Adoption Information
To request non-identifying information regarding a North Carolina adoption, contact the local adoption agency. In the event that the local adoption agency is unknown, a request may be submitted for the name of the local adoption agency at which time the local adoption agency should be contacted for the requested information. Non-identifying information will only be provided to adoption agencies not the public. Please
note North Carolina does not have information available for years prior
to 1936. The written request should be submitted to:
