To request information from the North Carolina Division of Social Services, please refer to the following:
Assistance
Verifications for Out of State Agencies
If you are requesting out-of-state verification of assistance, please do not send confidential information via email. You may obtain verification by calling 919-733-9370 or faxing a request to 919-715-5457. The request should be on agency letterhead and must include:
- Name
- Date of Birth
- Social Security Number
- Programs included in request
- Your agency's contact information
Responsible Individuals Requests
If you are requesting information in response to an individual’s application to become a foster or adoptive parent (Adam Walsh), or you are considering an individual for employment in a childcare setting please submit the DSS-5268. Once completed please mail this form, with original signatures to:

Child Protective Services Requests

Non-Identifying Adoption Information
To request non-identifying information regarding a North Carolina adoption, contact the local adoption agency. In the event that the local adoption agency is unknown, a request may be submitted for the name of the local adoption agency at which time the local adoption agency should be contacted for the requested information. Non-identifying information will only be provided to adoption agencies not the public. Please
note North Carolina does not have information available for years prior
to 1936.The written request should be submitted to:
