PURPOSE OF REPORT: To track the placement of children in licensed family foster homes
FREQUENCY: Daily
SOURCE OF DATA: DSS-5094
CONTENT: This report shows, on a daily basis, the status of all
licensed family foster homes which are supervised by the county.
The children who resided in that facility during the month are listed
for each home, showing the beginning (and ending, if applicable)
dates of each placement.

REPORT NAME: Previous Month Foster Care Placement Record

TYPE OF REPORT: Case Management
PURPOSE OF REPORT: To track the placement of children in licensed family foster homes.
FREQUENCY: Daily
SOURCE OF DATA: DSS-5094
CONTENT: This report is essentially the same as the current month
report, but reflects data from the previous month. This report can
assist workers to identify any errors on the DSS-5094 which affect
payment. Error messages on the PQA020 Foster Care Payment Report
regarding non- compliance with licensing requirements can be checked
against this report. Workers can then make corrections via the DSS-5094.
Because the report is updated nightly, staff can make corrections
in the Child Placement and Payment System one day and check this
report the next day to see the effect of the corrections.

REPORT NAME: Foster Care Case Management Report

TYPE OF REPORT: ` Case Management
PURPOSE OF REPORT: To assist staff in scheduling re-licensing activities, and to keep staff informed of the status of the license.
FREQUENCY: Daily
SOURCE OF DATA: DSS-5015
CONTENT: The report includes the name of the foster parent, the
address and Facility ID#, the beginning and ending dates of the
license period and the licensed capacity, sex and age range of the
children for whom the home is licensed. The report is arranged so
the facilities which are next due (or overdue) to be relicensed
are at the top of the report, and newly licensed/relicensed are
at the bottom.

REPORT NAME: Case Manager’s Report

TYPE OF REPORT: Case Mangement
PURPOSE OF REPORT: To remind each case manager of case management activities which are due in the current month, due the next month, and overdue to assist in scheduling.
FREQUENCY: Monthly
SOURCE OF DATA: DSS-5094
CONTENT: One report is created for each case manager. It reminds
the worker of reviews and redeterminations that are due, lists clients
whose Foster Care or Adoptions Assistance benefits are terminated
and provides a reminder when a child is reaching the age of 18.
NOTE: Because the name of the case manager for this report is derived
from the child’s record in SIS and is dependent on the way
the name is entered, a case manager may get more than one report.
For instance, if the case manager’s name is entered as Smith,
J. on one record and Smith, J.L. on another record, two case manager’s
reports will be created because of the difference in the way the
name was entered.

REPORT NAME: Quarterly Foster Care Client Analysis

TYPE OF REPORT: Statistical
PURPOSE OF REPORT: To provide counties with a statistical analysis of their foster care cases.
FREQUENCY: Quarterly
SOURCE OF DATA: DSS-5094
CONTENT: This report is presented in three parts: Active cases,
Terminated Cases, and New Cases. The Active Cases part of the report
covers all the cases which were open at any time during the quarter
and reflects the status of the cases as of the last day of the quarter
or, for the terminated cases, the last day on which the cases were
open. The other two parts of the report are a subset of the Active
Cases report. They show, respectively, data for cases which were
active at some time during the quarter but which were closed prior
to the end of the quarter, and new cases which were opened at some
time during the quarter. The reports are presented with breakdowns
by race, sex and age in relation to the Plan Goal, the Placement
Reason, the Type of Authority, and the Termination Reason as reported.
The Time in Custody part of the report shows an average length of
time based on the span of time from the date of placement to the
last day of the quarter for the cases which are still open, and
from the date of placement to the date of termination for those
cases which were terminated during the quarter.

NAME OF REPORT: Quarterly Case List

TYPE OF REPORT: Case Management
PURPOSE OF REPORT: To assist the county to identify all active foster care cases
FREQUENCY: Quarterly
SOURCE OF DATA: DSS-5094
CONTENT: This report lists all the Foster Care for Children records
which were active, i.e.open in the Child Placement and Payment System
at any time during the report period. The data reflects the current
status as of the last day of the report period or, in the case of
records which were closed during the quarter, the data which was
current on the last day the case was open. The report is sorted
by the Plan Goal and includes the child’s name, SIS ID#, age,
placement type, authority reason, the number of continuous months
that the child had been in the care/custody of the county. the number
of continuous months that the child had been in a foster care living
arrangement, and the termination reason, as appropriate.

PURPOSE OF REPORT: To assist workers to identify children for whom the Title IV-E administrative activities time may be charged when completing the DSS-4263 Worker Daily Report.

CONTENT: This report lists and children who were identified on
the DSS-5094 form as eligible for Foster Care Assistance under Title
IV-E.

PURPOSE OF REPORT: To provide statistical information regarding the living arrangements of the children who have open records in the Child Placement and Payment System.
FREQUENCY: Quarterly
SOURCE OF DATA: DSS-5027
CONTENT: This report identifies the current living arrangement
of all children who are in the custody or placement authority of
the county DSS as of the last day of the quarter. This is an unduplicated,
point-in-time report and includes only those cases which were open
on the day that the report was created.

REPORT NAME: Barriers to Plan Achievement

TYPE OF REPORT: Case Management
PURPOSE OF REPORT: To group children who have the same barriers to plan achievement to enable staff to plan group activities directed toward the removal of these barriers.

CONTENT: The report identifies each child who has an open record
and shows both the primary and secondary barrier and the name of
the case manager.

NAME OF REPORT; Foster Care Payment Report
XPTR INDEX NAME: DHRPQ PQA020 FOSTER CARE PMT RPT
TYPE OF REPORT; Fiscal
PURPOSE OF REPORT: To enable counties to correct errors in making foster care maintenance claims prior to reimbursement and to obtain information regarding actual reimbursements.
FREQUENCY: This report is created twice each month. The first version
is run on the night of the fifth working day of the month which
corresponds to the final date for the submission of county requests
for reimbursement for all programs. The second version is run on
the night of the 20th of the month or the last working day before
the 20th of the month which corresponds to the date that state reimbursements
are made to counties for all programs.

CONTENT: The initial run of this report shows the Foster care claims made by the county and provides an opportunity to make corrections in the systems which influence the claims.
NAME OF REPORT: Placement Resources for Children

PURPOSE OF REPORT: To assist county staff to locate placement resources
for children other than those which are supervised by the county
departments of social services.

SOURCE OF DATA: Foster Care Facility Licensing system
CONTENT: This report provides a statewide listing of all facilities
which are licensed under DSS standards, other than those which are
supervised directly by the county DSS. The list is sorted by county
where the facility is physically located and includes facility licensing
information and the name and telephone number of the supervising
agency.

CONTENT: This report shows, on a monthly basis, (1) the number & percentage of children in State Fiscal Year Cohorts with substantiated
abuse/neglect who have subsequent substantiations, (2) the number
& percentage of children in State Fiscal Year Cohorts with a
plan of adoption who have become legally free, (3) the number & percentage of children in State Fiscal Year Cohorts with a plan
of adoption for whom an adoption has been finalized, and (4) monthly
caseload dynamics.

which were submitted by the worker during the previous month and
which passed the edits necessary for inclusion in the active data
base. A report on compliance with the time frame requirements for
beginning investigations after a report has been received, and the
completion of investigations is also included.

TYPE OF REPORT: Case Management
PURPOSE OF REPORT: To identify children who have been reported as abused or neglected and who have not received services.
FREQUENCY: Monthly
SOURCE OF DATA: DSS-5027 and DSS-4263
CONTENT: All records where Protective Services for Children - Investigation
and/or Protective Services for Children - Treatment have been added
to the Service Plan are compared to the Worker Daily Report of Services
Provided (DSS-4263) If no service activity has been reported for
these records over the last three months, the children’s names
are included on this list.
