Funded through the Mental Health Trust Fund this housing pilot has three full-time Housing Support Teams targeting the homeless population who are cycling through hospitals, treatment programs, jails and prisons. These teams are specialized, knowledgeable, staff focusing on housing outcomes and are available 24 hours/7 days a week. They provide services and interventions necessary for previously homeless individuals who have severe and persistent mental illness to maintain their housing. Once the individual served is linked to a community-based enhanced service provider, the Housing Support Teams may continue to provide limited services related to housing needs.
