Why do I need to fill out this form?

Phone Usage Policy. This policy provides guidelines for the acceptable use of the Department's phone systems and cellular/mobile phones. Employees must use the phone system responsibly. All employees should follow guidelines used for local and long distance calls as well processes for reimbursements.
