Why do I need to fill out this form?
The W-4 form allows your employer to withhold the correct amount of Federal
income tax from your salary.
Be sure to sign your form before you turn it in!
What happens if I do not identify a number of allowances?
Your tax will be based on zero deductions.
What happens if I have not submitted my form in
enough time for payroll to deduct the correct number of
allowances?
Your tax will be based on zero deductions for your
first paycheck. The number you entered will be used
as soon as your W-4 is submitted and processed.
May I change my number of allowances? If so, how
do I do that?
Yes, you may change your number of allowances at any
time during the year. Submit a new signed and dated
W-4 form to your HR office.
