CAP-MR/DD Unlicensed AFL Health and Safety On-Site Review form is used by the LME(s) to document the annual review/monitoring of unlicensed AFL settings.
Prioritization Tool and Instructions - Individuals who seek CAP-MR/DD Waiver services are evaluated *by the LME* for their potential eligibility and then are prioritized according to needs. This document outlines the procedure in which this process is completed.

LME Waiver Transition Letter | Instructions Notifies participants if assigned to Supports Waiver or Comprehensive Waiver. Used by the LME and Case Management Agency to explain the process of submitting the Waiver Transition Letter to participants. The Case Manager meets with the participant/family members to provide information regarding the transition process.
