where ### is area program code, MMM is month,
e.g. JUN for June and JUL for July,and YYYY is year, e.g. 2003

Files will remain available at SIPS for 30 days after creation.

Notes:
  1. November 7, 2003: Block size has changed. New file is FB-500-4000

  2. November 7, 2003: The file layout has changed to accommodate current and anticipated changes due to HIPAA. If you wish to receive a new file layout, please contact Adam Holtzman, at Adam.Holtzman@dhhs.nc.gov.

  3. November 7, 2003: Versions of the CAPMR Paid Claims Access Application that were used through October 2003 will not properly accept the new file structure and MUST be replaced by an updated version of the Paid Claims Application available from DMH/DD/SAS.
