The LME Waiver Transition letter notifies participants if assigned to Supports Waiver or Comprehensive Waiver. Used by the LME and Case Management Agency to explain the process of submitting the Waiver Transition Letter to participants. The Case Manager meets with the participant/family members to provide information regarding the transition process.
