SA/MH Block Grant Review Information The Division announces a new Statewide Crisis Services RAD (Reactive Attachment Disorder) Clinical Guidelines DMH/DD/SAS has received requests recently from providers seeking Medicaid Paid Claims data to complete their required Medicaid Cost Reports. The Division of Medical Assistance (DMA) is the appropriate agency to contact to request that data. These links take you to a Provider Mental Health Claims Data Request Form in .pdf and Excel formats in the DMA website. Upon submission of the request form, EDS will be able to generate the data for each unique Medicaid provider number for claims adjudicated within the past 24 months. For those services billed through the LME, EDS will not be able to extract the data based upon the attending provider number. The provider will need to contact the LME for their claims data report.
