North Carolina's new state brand for Aging and Disability Resource Centers is "Community Resource Connections for Aging and Disabilities" (CRC). This brand reflects one of the main goals of this initiative, "community" collaboration. Through this collaboration, agencies and organizations within the community work together to provide information about, assistance with, and access to services for individuals who are aging or have a disability. "Resource Connections" denotes that individuals will be connected to resources for long-term services and supports, and "for Aging and Disabilities" identifies the individuals to be served through the CRC.

Community Resource Connections for Aging and Disabilities is North Carolina's implementation of an Aging and Disability Resource Center (ADRC) grant. This grant is a cooperative effort of the Administration on Aging (AoA) and the Centers for Medicare & Medicaid Services (CMS). This program was developed to assist states in their efforts to create a coordinated system of information and access for all people seeking long-term support and services, and to minimize confusion, enhance individual choice, and support informed decision-making.

CRC is considered to be one of the primary vehicles to modernize long-term care systems, particularly in relation to helping people avoid unnecessary institutionalization by supporting individuals of all disabilities and incomes to make informed, cost-effective choices. In North Carolina, the initiative is not about adding new services but rather building on existing community infrastructure and realigning systems and processes.
