Connect the Dots

There are numerous programs and grant initiatives within the Department of Health and Human Services that relate to long-term services and supports. Recognizing the need to make connections and find common threads among these activities, Connect the Dots was formed.

Connect the Dots is a forum for lead staff from DHHS divisions and offices that provides an opportunity to:

  • learn from one another about program and initiative activities,
  • identify opportunities for collaboration,
  • maximize resources,
  • identify ways to sustain and build on these efforts in moving toward a more cohesive long-term services and supports system

An LTS Action Plan is a product of the Connect the Dots forums. This plan outlines major initiatives and programs related to long-term services and supports, and identifies connections among these activities. The LTS Action Plan is "fluid" with periodic updates on current activities and additions of new initiatives.
