The Office of Long-Term Services and Supports is working with other divisions and offices to improve information technology so that it increases access to services, facilitates data sharing, and is consumer-friendly. This project is funded primarily through the Systems Transformation Grant received by the Department of Health and Human Services
from the Centers for Medicare & Medicaid Services. Through this project, state and local staff, providers, consumers and family members will be better able to access information and to communicate with one another through an Internet-based system.
NCcareLINK is available to provide initial information about services and supports, and the Medicaid Uniform Screening Tool will help determine appropriate Medicaid services for individuals. Future plans include a Web-based system that Medicaid case managers can use to do assessment and planning. This same system will have a portal for consumers and family members to learn about their health conditions and improve self-management of their conditions.
