Through grants to community networks, HealthNet supports communities’ efforts to work together to share resources in finding “medical homes” for the uninsured. Through HealthNet, a low-income or uninsured person has easy access to available health care services in his or her area. The HealthNet model is linked to Community Care of NC’s existing networks of health care providers in each region of the state. Networks include community physicians, hospitals, health departments, community and rural health centers, and other community-based organizations affiliated with CCNC. Individual health care providers and safety net organizations that would like to partner with the local HealthNet network or want help with planning and organizing a HealthNet network in their community should contact CCNC’s Community Care Coordinator for the county/region.
