Printable Interview Forms
These interviews were designed to assist System Users in evaluating effectiveness of treatment. The instruments were built on research findings and field practice. The Initial and Update Interviews do not cover all consumer problems that may warrant clinical intervention, but they do cover a majority of standard problems known to affect the populations. The Initial Interview is designed to gather information on consumer demographics and pretreatment behaviors. The Update Interview is designed to gather information on consumer behavioral or status changes.
