Office of Communications: Web Request Form

Please use this form for all content edit requests for the NCDHHS.gov website only

  • Fill out the form and provide all required information.
  • All instructions and attachments must be final and approved before submission.
  • Please submit once and wait for confirmation.
  • You will receive an email when the request is complete.

All documents to be added to the site must include a signed PA-2 form

  • Download and fill out the PA-2 Form.
  • Email the PA-2 form and the original editable document to pa2@dhhs.nc.gov  
  • Upon approval, you will receive the signed PA-2 form.
  • Fill out and submit a web request form and email the signed PA-2 form and your document to webservices@dhhs.nc.gov 

Document Accessibility

After April 24, 2026, inaccessible documents cannot be posted on public-facing websites. This includes revised versions of existing documents.  

Before creating a PDF, consider not publishing content in PDF format and instead contact your web content manager to create an HTML web page or another content type.

Questions to ask if you decide the content needs to stay in PDF format include:  

  • Is offline access required? - Keep it a PDF.  
  • Is it a form that must be printed, completed by hand, and mailed? - Keep it a PDF.
  • Do you need an exact replica of a publication (e.g., magazine or report)? - Keep it a PDF.
  • Is the document password-protected? - Keep it a PDF.
  • Is there a legal requirement to keep in PDF format? - Keep it a PDF.

If a document—most often a PDF—must be created, you are responsible for ensuring it is accessible. The easiest way to do this is to apply accessibility best practices in Microsoft Word before converting the file to a PDF. Microsoft applications such as Word, Excel, and PowerPoint include built-in accessibility tools to help you create accessible documents from the start.
