About the NC ROOTS Hubs

NC Rural Organizations Orchestrating Transformation for Sustainability (ROOTS) Hubs are regional, locally governed networks that coordinate services and lead implementation of NC Rural Health Transformation Program (RHTP) initiatives. Working with community partners, each Hub will help advance efforts across primary care, behavioral health, workforce development, value-based payment and digital health — tailoring strategies to the unique needs of their respective regions.

Meet the ROOTS Hub Leads

The selected NC ROOTS Hub Leads will represent each of the North Carolina Medicaid regions and will coordinate care and implement NCRHTP initiatives. Impact Health is an independent 501(c)3 formed during the creation of North Carolina’s Healthy Opportunities Pilot (HOP) program. While they do not provide direct services, Impact Health works with more than 60 trusted community-based organizations to deliver high-quality, culturally appropriate services in the western North Carolina region. Impact Health serves as a bridge between participants, service providers and private health plans, and support essential program functions, including eligibility screening, service referrals and Medicaid billing.

Trillium Health Resources is an NC Medicaid Tailored Plan and Managed Care Organization (MCO) that manages serious mental health, substance use, traumatic brain injury and intellectual/developmental disability services in North Carolina. Trillium works with their provider network to strengthen foundations of well-being and help deepen connections between residents and their communities across central and eastern North Carolina.

Vaya Health is a public NC Medicaid Managed Care Organization (MCO) that prioritizes whole-person health. Vaya manages Medicaid, federal, state and local funding to connect people to health care services and supports in western and central North Carolina. Vaya's specialty is helping individuals with needs related to mental health, substance use disorders, intellectual/developmental disabilities and traumatic brain injuries. To manage prevention, treatment and crisis services, Vaya partners with more than 4,000 health care organizations and practitioners across the state.

UNC Hospitals is a public academic medical center providing patient care, educating health care professionals and advancing medical research in partnership with the UNC School of Medicine. With more than 1,000 beds, the medical center on the campus of the University of North Carolina-Chapel Hill focusing on a wide range of specialty care, including women’s and children’s health and cancer treatment. UNC Hospitals’ mission is to improve the health and well-being of North Carolinians by providing leadership and excellence in the interrelated areas of patient care, education and research.

Access East, Inc. is a comprehensive care management provider and participant in North Carolina’s Healthy Opportunities Pilot (HOP) program. Access East partners with health care organizations, including primary-care practices, Federally Qualified Health Centers (FQHCs), community health centers, local health departments, safety-net and social service organizations and hospitals, to create better access to quality care for eastern North Carolina residents, including the chronically ill, underinsured and uninsured.

Following a regional needs assessment, the NC ROOTS Hub Leads will establish local networks of partner organizations focused on strengthening the rural health care infrastructure. Over the next five years, tailored to their regions’ needs, NC ROOTS Hubs will support rural communities by: 

  • Enhancing care coordination across regions
  • Leveraging digital tools to improve outcomes
  • Expanding access to primary care
  • Strengthening health care through workforce development
