NC Incident Response Improvement System (IRIS)
The Incident Response Improvement System (IRIS) is a web based incident reporting system for reporting and documenting responses to Level II and III incidents involving consumers receiving mental health, developmental disabilities, traumatic brain injury, and substance use services.
Register for NCID
Before accessing IRIS, make sure you have an active NCID account. Registering for an NCID is the first step and should be completed before attempting to log in to the system.
Access IRIS
Use the live IRIS system to enter a new incident report or view or edit an existing report.
