What Are Evidence-Based Health Promotion/Disease Prevention Programs?
Evidenced-Based health promotion/disease prevention (EB HP/DP) programs include education and activities that support healthy lifestyles and promote healthy behaviors among older adults aged 60 and older. Evidence-based means the program was studied and shown to be effective at creating a desired result.
There are several different evidence-based programs offered in NC:
- Programs aimed at getting people more active:
- Programs to help manage emotional or caregiving challenges:
- Programs that help prevent older adults from falling:
- Programs that help people manage their chronic conditions:
Who Qualifies for EB HP/DP Programs?
EB HP/DP programs are limited to individuals 60 years of age and older. Individuals should be independent or able to manage their own care needs. Some organizations offer these programs using other funding which will change the eligibility criteria and allow those under 60 to participate.

EB HP/DP programs are offered by a variety of partners. The NC Center for Health and Wellness at UNC Asheville offers convenient access to many popular programs through its website Healthy Aging NC.
